Job Summary:

Pacific Coast Community Resources is seeking a skilled Nurse Care Coordinator to join our team. As a key member of our healthcare team, you will provide high-quality care and support to individuals with diverse abilities, promoting independence, learning, and self-expression.

Key Responsibilities:

  • Provide nursing care and support within the scope of your practice, collaborating with physicians, therapists, and other healthcare professionals.
  • Develop and implement individualized program plans to address the total needs of each supported person.
  • Communicate effectively with team members, families, and community resources to ensure seamless care and support.
  • Monitor the health and safety of supported persons, implementing preventive measures as needed.
  • Participate in planning meetings, preparing for and following up on each meeting as required.
  • Complete personnel forms, including time sheets, expense claim sheets, and mileage sheets.
  • Be available for additional shifts as required.

Requirements:

  • Previous experience in a healthcare setting or recent graduation from a nursing program with a multidisciplinary approach.
  • Experience working with youth with significant substance use/addiction and mental health challenges.
  • Experience in Individualized Program Plans (IPP), Person-Specific Planning (PSP), and Health Care Planning (HCP).
  • Valid Class 5 driver's license; Class 4 driver's license preferred.
  • Ability to make clinical judgments based on current nursing standards.
  • Ability to self-evaluate performance critically.
