About the Role

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Island Health. As a key member of our primary care team, you will be responsible for providing high-quality, patient-centered care to individuals who face systemic barriers within the healthcare system.

Key Responsibilities:

  • Provide comprehensive primary care services to patients, including health assessments, diagnosis, and treatment of acute and chronic conditions.
  • Develop and implement individualized care plans that address the unique needs and circumstances of each patient.
  • Collaborate with other healthcare professionals, including physicians, social workers, and registered nurses, to ensure seamless care coordination and support.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practices in primary care.

Requirements:

  • Master's degree in Nursing or related field.
  • Current registration with the College of Registered Nurses of British Columbia.
  • Experience working with vulnerable populations and in primary care settings.
