Key Responsibilities:
- Provide comprehensive primary care services to patients, including health assessments, diagnosis, and treatment of acute and chronic conditions.
- Develop and implement individualized care plans that address the unique needs and circumstances of each patient.
- Collaborate with other healthcare professionals, including physicians, social workers, and registered nurses, to ensure seamless care coordination and support.

Requirements:
- Master's degree in Nursing or related field.
- Current registration with the College of Registered Nurses of British Columbia.
