Are you a dedicated and compassionate Registered Practical Nurse (RPN) looking for a challenging role in the Durham area?

We are seeking an experienced RPN to join our team at RhynoCare, where you will play a vital role in providing high-quality care to our residents.

As a Care Coordinator RN with RhynoCare, your primary responsibility will be to provide nursing care in accordance with established policies, physician's orders, and the resident plan of care. You will work closely with the healthcare team to ensure that our residents receive timely and accessible admission, as well as comprehensive orientation to our home and services.

Your duties will include:

  • Assessing, planning, implementing, and evaluating nursing care to meet the physical, psychological, emotional, spiritual, cultural, and social needs of our residents
  • Ensuring timely and safe resident transfers and discharges
  • Managing medications and narcotics safely and effectively
  • Collaborating with the healthcare team to develop and implement individualized care plans

To succeed in this role, you must have:

  • A Bachelor of Science – Nursing (BScN), Bachelor of Nursing (BN), or equivalent degree
  • Current registration as a Registered Practical Nurse with the College of Nurses
  • Basic Cardiac Life Support (BCLS) certification
  • Strong written and verbal English communication skills
  • Effective time management and organizational skills
  • The ability to work collaboratively as part of a multidisciplinary team
