Care Coordinator

We are seeking a skilled Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in assessing and determining patient care needs, providing access to community services, and engaging with patients, caregivers, and healthcare practitioners.
Key Responsibilities- Assess patient care needs and determine eligibility for services
- Develop individual care and service plans in collaboration with patients and their families
- Link patients with service providers and coordinate care plan delivery
- Establish a helping relationship with patients and their families, respecting their values and preferences
- Balance patient needs with available resources, ensuring effective care delivery
- Membership in good standing with the applicable regulatory body (College of Nurses of Ontario, College of Physiotherapists of Ontario, College of Occupational Therapists of Ontario, Ontario College of Social Workers and Social Service Workers)
- Recent experience in community health or a related field
- Knowledge of the healthcare delivery system and community resources
- Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
- Effective time management, prioritization, and organizational skills, with the ability to work independently and collaboratively in a busy multidisciplinary environment
- Established ability to accurately complete required documentation, reports, and forms
- A valid driver's license and access to a reliable vehicle
- Proficient in a Windows environment
- Experience working with diverse patient groups, such as multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics
- Case management experience or recent related community experience
- Ability to speak French or another second language
