We are seeking a skilled Registered Nurse to join our team as a Rapid Response Nurse. In this role, you will provide high-quality care to patients in their homes, focusing on the transition from acute care to home care for complex and chronically ill adults and medically fragile children.
Key Responsibilities:- Conduct in-home nursing visits within 24 hours of hospital discharge for patients with complex/chronic diseases.
- Perform clinical assessments, confirm hospital discharge plans, and reconcile medications.
- Utilize a 'teach back' approach to educate patients and caregivers on care plans, treatments, and symptom management.
- Initiate communication with primary care providers and ensure follow-up within 7 days.
- Refer patients to community programs as needed.
- Registered Nurse in good standing with the College of Nurses of Ontario.
- Minimum 3-5 years of clinical experience as a Registered Nurse.
- Recent experience in Paediatrics, Chronic Disease Management, Medicine, or Cardiology.
- Superior clinical assessment skills and ability to work independently.
- Strong critical thinking and problem-solving skills.
- Effective interpersonal and communication skills.
- Ability to provide health coaching and disease-specific education.
- Advanced oral and written proficiency in English.
- Valid driver's license and access to a vehicle.
- Valid Emergency First Aid Certification.
- Experience working with diverse patient groups.
- Case management experience or recent related community experience.
- Ability to speak French or another second language.

- Hourly wage of $41.29 to $51.13 as per the ONA Collective Agreement.
