We are seeking a highly skilled and compassionate Intake Coordinator to join our team at Home and Community Care Support Services. As an Intake Coordinator, you will play a vital role in assessing patient needs, determining eligibility for services, and developing individual care and service plans.

Key Responsibilities:
  • Collaborate with patients and their families to assess care needs and develop individual care and service plans
  • Link patients with service providers and coordinate care plan delivery
  • Establish a helping relationship with patients and their families, balancing their needs and choices with available resources
  • Work effectively with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics
Requirements:
  • Registered Practical Nurse in good standing with the College of Nurses of Ontario
  • Relevant experience in long-term care homes or healthcare and/or in social services
  • Experience working with seniors and/or physically disabled individuals
  • Understanding of issues and priorities within the healthcare sector
  • Knowledge of tools, systems, and databases used in healthcare service delivery and management
  • Practical knowledge and understanding of relevant legislation regarding the provision of healthcare services, privacy, and other related areas
  • Effective planning, organizing, and evaluation skills to manage multiple patients and provide information reports upon request
  • Ability to accurately complete required documentation, reports, forms, and recommendations
  • Strong interpersonal and communication skills to work with diverse patient groups with varying levels of comprehension and language capability
  • Effective listening and strong facilitation skills to work with patients and their families, caregivers, or substitute decision-makers (SDMs)
  • Ability to communicate using various software programs to clearly articulate patient status
  • Strong collaboration skills to participate in projects and committees within the organization
  • Working knowledge of computer software (email, internet) and Microsoft Office applications (Word, Excel, PowerPoint, Outlook)
  • A valid driver's license and access to a reliable vehicle
  • Advanced oral and written proficiency in English is essential
