Key Responsibilities:- Assess patient needs and determine eligibility for services
- Develop and implement individual care and service plans
- Link patients with service providers and coordinate care plan delivery
- Establish a helping relationship with patients and their families
- Balance patient needs and choices with available resources
- Registered Practical Nurse in good standing with the College of Nurses of Ontario
- Relevant experience in long-term care homes or healthcare and/or in social services
- Experience working with seniors and/or physically disabled individuals
- Understanding of issues and priorities within the healthcare sector
- Knowledge of tools, systems, and databases used in healthcare service delivery and management
- Practical knowledge and understanding of relevant legislation
- Effective planning, organizing, and evaluation skills
- Ability to accurately complete required documentation and reports
- Strong interpersonal and communication skills
- Ability to deal with demanding interpersonal situations
- Working knowledge of computer software and Microsoft Office applications
- A valid driver's license and access to a reliable vehicle
- Advanced oral and written proficiency in English
