We are seeking a highly skilled and compassionate Intake Coordinator to join our team at Home and Community Care Support Services. As an Intake Coordinator, you will play a vital role in ensuring that patients receive the care and support they need in a timely and efficient manner.

Key Responsibilities:
  • Assess patient needs and determine eligibility for services
  • Develop and implement individual care and service plans
  • Link patients with service providers and coordinate care plan delivery
  • Establish a helping relationship with patients and their families
  • Balance patient needs and choices with available resources
Requirements:
  • Registered Practical Nurse in good standing with the College of Nurses of Ontario
  • Relevant experience in long-term care homes or healthcare and/or in social services
  • Experience working with seniors and/or physically disabled individuals
  • Understanding of issues and priorities within the healthcare sector
  • Knowledge of tools, systems, and databases used in healthcare service delivery and management
  • Practical knowledge and understanding of relevant legislation
  • Effective planning, organizing, and evaluation skills
  • Ability to accurately complete required documentation and reports
  • Strong interpersonal and communication skills
  • Ability to deal with demanding interpersonal situations
  • Working knowledge of computer software and Microsoft Office applications
  • A valid driver's license and access to a reliable vehicle
  • Advanced oral and written proficiency in English
