Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse to join our team at Fraser Health. As a key member of our community health team, you will provide high-quality nursing care to clients in a community setting.

Key Responsibilities
  • Assess and monitor clients' physical, emotional, and social needs, and develop individualized care plans to meet their unique requirements.
  • Provide supportive care and education to clients and their families, promoting independence and self-management of health conditions.
  • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions between healthcare services.
  • Administer medications and provide medication reminders, as needed, in accordance with established policies and procedures.
  • Assist clients with activities of daily living, such as bathing, dressing, and toileting, and provide emotional support and companionship.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance client care and safety.
Requirements
  • Grade 12 diploma, plus graduation from an approved program of Practical Nursing, or an equivalent combination of education, training, and experience.
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Certificates in CPR Level C or equivalent and First Aid.
  • Valid BC Driver's License and access to a personal vehicle for business-related purposes.
Preferred Qualifications
  • Experience working in a community health setting, preferably with Fraser Health.
  • Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare teams.
  • Ability to prioritize tasks, manage time, and maintain accurate records.
