Job Title: Clinical Nurse Coordinator for Liver Disease Management

Job Summary:

The University Health Network is seeking a highly skilled Clinical Nurse Coordinator to join our team in the Liver Clinic. As a key member of our multidisciplinary team, you will play a pivotal role in enhancing the quality of life for patients affected by metabolic-associated fatty liver disease (MAFLD) through coordinated care, education, and advocacy.

Key Responsibilities:

  • Provide comprehensive education to patients and their families on lifestyle modifications, dietary guidelines, medication adherence, and potential complications.
  • Facilitate access to resources and support services for patients.
  • Maintain accurate and timely documentation of patient interactions and intervention plans.
  • Perform Fibroscan tests.
  • Collaborate with staff hepatologists to optimize patient care pathways and outcomes.
  • Participate in quality improvement initiatives and data analysis.
  • Direct care responsibilities: performing venipuncture, IV insertion, albumin transfusions, paracentesis, and thoracentesis procedures, wound care, recovering patients post-procedures, and liaising with clinicians regarding patient care.

Qualifications:

  • Completion of a university, college, or other approved nursing program.
  • Current Basic Cardiac Life Support (BCLS) certification.
  • Current certificate of competency from the College of Nurses of Ontario.
  • Physical Health Assessment Course preferred.
  • Completion of the ECG interpretation course is an asset.
  • Minimum of 2 years of clinical experience in hepatology, gastroenterology, or a related field.
  • Certification in hepatology nursing or related specialty (preferred).
  • Experience with phlebotomy, IV insertion, insertion of and access to vascular access lines, and administration of blood products.
  • Demonstrated ability to exercise initiative and good judgment with the ability to multi-task.
  • Ability to work as an integral member of a team.
  • Excellent independent decision-making and problem-solving skills.
  • Excellent organizational and time management skills.
  • Excellent verbal, interpersonal, and written communication skills.
  • Patient teaching skills.
  • Ability to work effectively as a member of a multidisciplinary team.
  • Ability to work under pressure.
  • Ability to work effectively in a fast-paced environment to meet the needs of a changing population.
  • Ability to utilize information technology.
  • Ability to meet the physical demands of the job.
