Mental Health Case Coordinator Position

Vancouver Coastal Health is seeking a skilled Mental Health Case Coordinator to join their team in Richmond, BC. As a key member of the multidisciplinary mental health team, this individual will provide psychosocial assessments, treatment, and case coordination services to patients and their families.
Key Responsibilities- Perform intake and discharge functions, facilitating patients' planned utilization of community services and providing liaison and service networking with other agencies.
- Provide treatment services, including crisis intervention, family work, individual counseling, and group therapy, according to various theoretical models.
- Participate in case conferences with patients, family, service providers, and members of the multidisciplinary team, and provide consultative and direct services to patients/families and staff regarding community resources.
- Prepare and maintain patient records, maintain workload statistics, and participate in team meetings, committees, continuing education, community initiatives, and quality improvement activities.
- Bachelor's degree in behavioural sciences.
- Two years' recent related experience in a mental health setting, or an equivalent combination of education, training, or experience.
- Valid BC Driver's License, with local area travel requiring the use of a personal vehicle.
