Job Summary:

We are seeking a compassionate and skilled Registered Practical Nurse (RPN) to join our community nursing team. As an RPN, you will play a vital role in providing high-quality care to patients in their homes, ensuring they receive the best possible treatment and support.

About the Role:

This is a full-time position that requires flexibility and a strong work ethic. You will be responsible for visiting patients in various settings, working with RNs for more complex cases, and collaborating with patients of all ages to develop personalized care plans. Your exceptional communication skills and ability to build rapport with patients will make a significant difference in their lives.

What You'll Do:

  • Participate in scheduled visits to assess patient needs and provide comprehensive care.
  • Develop and implement care plans tailored to individual patients' requirements.
  • Maintain accurate and up-to-date patient records, reflecting their progress and any necessary adjustments to their care plan.
  • Engage patients through empathy and understanding, fostering trust and cooperation.

Requirements and Qualifications:

  • Current registration as a Registered Practical Nurse with the CNO.
  • A valid driver's license and reliable access to a vehicle.
  • Experience in delivering community care is an asset, but not mandatory.
  • Reliability, flexibility, and a willingness to travel to patient residences within a designated area.
