Care Coordinator for Long-Term Care Residents

The Steeves and Rozema Group is committed to providing exceptional care to our residents. We are seeking a skilled Nurse Practitioner to join our team as a Care Coordinator. This role involves assessing, diagnosing, and developing therapeutic plans of care for our residents.
About the Role
This full-time position requires a qualified Nurse Practitioner with experience in long-term care settings. You will work closely with our healthcare team to ensure that our residents receive high-quality care and services tailored to their individual needs. As a key member of our interdisciplinary team, you will contribute to chart documentation, participate in chart reviews, and case conferences.
Key Responsibilities:
• Perform comprehensive assessments, diagnose conditions, and develop treatment plans in accordance with CNO standards for NP's
• Collaborate with primary care providers, family members, and other healthcare professionals to ensure continuity of care
• Prescribe medications, order medical tests, and provide health education/counselling to residents and families
• Maintain accurate and confidential records of resident care and participate in quality improvement initiatives
• Provide leadership and guidance to support staff to enhance best practice outcomes
Requirements:
We are looking for a qualified Nurse Practitioner who meets the following requirements:
• Current registration with the College of Nurses of Ontario (CNO) and extended class certification
• Membership with the Nurse Practitioners' Association of Ontario (NPAO)
• Experience in a long-term care setting preferred
• Strong communication, interpersonal, and organizational skills
• Ability to work collaboratively within an interdisciplinary team
• Superior time management and problem-solving skills
• Knowledge of risk and quality management principles
Benefits:
The Steeves and Rozema Group offers a competitive salary range of $100,000 - $130,000 per year, depending on experience. Our benefits package includes dental care, disability insurance, extended health care, flexible schedule, life insurance, on-site parking, and vision care.
