Job Summary

The Case Coordinator provides comprehensive assessments to clients, ensuring a high-quality client experience. This role involves developing care plans in consultation with clients and caregivers, managing care budgets, and navigating clients to appropriate resources.

Key Responsibilities
  • Maintains competency in standardized assessment tools, such as the interRAI CHA.
  • Completes client assessments in the home using conversation-based methodology.
  • Develops and conducts comprehensive therapeutic rapport.
  • Utilizes technology to complete assessments in the home.
  • Identifies and prioritizes environmental risks for client health and safety.
  • Develops comprehensive care plans, interpreting CAPs and care planning.
  • Facilitates access to client-identified services to support care plans and goals.
  • Makes referrals to community and health resources as required.
  • Consults with partners in care service plans to ensure shared understanding of client needs.
  • Monitors client progress and adjusts care plans as needed.
  • Reviews client care plan expenditures and supports clients to manage within budgets.
  • Accesses and reviews assessments via the IAR.
  • Collects and reports relevant data to Manager when necessary.
  • Adheres to prescribed privacy policies and standards for Community Support Service agencies.
Education, Designations, and Experience
  • University Degree or College Diploma in social work and nursing field.
  • Minimum of 3 years related experience in home and community care, including experience in case management.
  • Broad working knowledge of community resources, including services and programs within defined catchment area.
  • Proven experience in building external and community relationships.
  • Demonstrated experience in completing validated common assessment tools and maintaining competency.
  • Knowledge and skills in comprehensive assessment and ability to maintain proficiency.
  • Computer proficiency in Windows OS and Microsoft Office programs.
Working Conditions and Physical Demands

This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying, bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in this role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
