Our Supportive Housing department provides a variety of housing, community, and supports-based services to adults & seniors who have struggled with homelessness, mental health and/or addiction-related challenges.

Job Description:
The Clinical Counsellor is a key member of our Supportive Housing team, responsible for providing emotional support and feedback to individuals in the program, facilitating their participation in the program, and developing and implementing service plans and/or coordinated care plans.

Responsibilities:
- Interact with individuals in the program in person, providing emotional support and feedback and encouraging and facilitating their participation in the program.
- Perform psychosocial and/or psychological assessments.
- Develop and implement service plans and/or coordinated care plans.
- Provide service/care coordination, system navigation, short-term and long-term planning, and follow-up support.
- Provide preliminary assessment of risk in crisis situations, consult and devise an intervention and action plan, and provide crisis intervention.
- Provide clinical and/or supportive counseling to clients utilizing a wide range of psychotherapeutic approaches that best meet the clients' needs and circumstances.
- Participate in case conferences and clinical meetings to ensure the coordination of services and a continuum of care, as well as maintaining relationships with partner organizations.
- Link clients to various community resources and facilitate appropriate referrals to other support services as required.
- Support clients with navigating health systems (hospitals, community health centers, primary care).
- Accompany clients as required to appointments that may require therapeutic support, such as medical appointments, court hearings, community engagements, etc.
- Facilitate or lead therapeutic and/or psychosocial support groups as required.

Working Environment:
- Work Setting: Onsite with limited opportunities to work remotely.
- Salary: $69,414.13.
