Job Description

We are seeking a highly skilled and compassionate LPN/RPN/RN to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and support in a dynamic and fast-paced environment.

Key Responsibilities:
  • Provide direct patient care and support to individuals with psycho-social needs
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with interdisciplinary teams to ensure comprehensive and coordinated care
