Patient Care Specialist

We are seeking a highly skilled and compassionate Patient Care Specialist to join our team at SE Health. As a key member of our care team, you will play a vital role in delivering high-quality patient care and making a meaningful difference in the lives of our clients.
About the Role:
- As a Patient Care Specialist, you will be responsible for completing comprehensive client assessments, developing care plans, implementing and evaluating care based on best evidence/practices and on client's unique needs.
- You will work collaboratively with other care team members and support functions to ensure seamless care transitions and optimal client outcomes.

Requirements:
- A current Certificate of Competence with the College of Nurses of Ontario.
- Availability to work days, evenings, nights, and every other weekend.
- Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
- A willingness to attain required SE Health certifications.
- A current CPR certificate is required.
