We are presently seeking an enthusiastic individual to join our high-functioning interprofessional team as a Nurse Practitioner (NP) in a Long-term Care (LTC) setting.
Key Responsibilities:- Deliver Comprehensive Health Services: Provide primary health care to residents in LTC, managing acute and chronic medical conditions, therapeutic management, health promotion, and disease/injury prevention.
- On-Call Coverage: Provide on-call coverage for duties and schedules as required.
- Clinical Leadership: Provide clinical leadership to LTC team members, centered around resident care needs, including resident admission/discharge, hospital discharge planning, palliative and end-of-life care, pain management, dementia care, and responsive behavior management.
- Health Assessments: Conduct health assessments using various tools and techniques based on resident needs.
- Collaboration and Communication: Collaborate and engage with residents and family members to develop person-centered care plans, synthesizing health information using critical inquiry and clinical reasoning to formulate a diagnosis, identifying health risks and states of health/illness.
- Interventions and Therapeutic Management: Select appropriate interventions from a range of non-pharmacological and pharmacological interventions to restore or maintain resident functional, physiological, and mental stability to achieve optimal health.
- Quality Improvement and Research: Participate in quality improvement and research initiatives, collaborating with leaders and community health services to promote continuity of resident care across the continuum.
- Registration and Certification: College of Nurses of Ontario registration in the Extended Class, Bachelor's degree in Nursing (BScN), and Master's degree in Nursing (MSN) with a Nurse Practitioner specialization preferred.
- Membership and Insurance: Membership with Nurse Practitioners' Association of Ontario (NPAO) (preferred) and acquired professional liability insurance.
- Gerontological Certification: National certification in gerontology is an asset.
- Communication and Interpersonal Skills: Excellent written skills and report writing for a clinical setting, ability to communicate with clients in a non-judgmental manner, and practicing active listening skills to understand client needs and coaching of clients as collaborative partners in their health care.
- Certifications and Checks: Basic CPR certification required, valid current Vulnerable Sector Check (VSC), and other immunizations as mandated by the Ministry of Long-term Care (MoLTC).
