Job Summary

Halton Healthcare is seeking a skilled Registered Practical Nurse to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care in a dynamic and rapidly changing environment.

Key Responsibilities
  • Provide direct patient care to complex transitional care populations, including assessment, planning, implementation, and evaluation of patient care.
  • Collaborate with interdisciplinary teams to develop and implement individualized patient care plans.
  • Administer medications and treatments as prescribed by physicians, and monitor patient responses to ensure optimal outcomes.
  • Conduct physical assessments, including vital signs, and document findings in patient records.
  • Provide wound care, enteral feeding, and maintenance of IV lines as required.
  • Utilize electronic documentation systems to record patient information and communicate with healthcare teams.
  • Participate in quality improvement initiatives to enhance patient care and safety.
Requirements
  • Current registration with the College of Nurses of Ontario.
  • Current Heart & Stroke Approved BLS Provider Card upon hire.
  • Experience working with and advocating for the elderly.
  • Medication administration certification and experience.
  • Physical assessment skills.
  • Wound care expertise.
  • Enteral feeding experience.
  • Maintenance of IV lines.
  • Experience with electronic documentation.
  • Satisfactory employment and attendance record.

  • Experience in a dynamic, rapidly changing environment.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a team environment.
  • Flexibility to work various shifts, including evenings and weekends.
