CarePartners, a leading home health care provider in Ontario, is seeking a dedicated Home Health Care Professional to join their team. This full-time role is based in the McDougall, McKellar, and Carling area and offers a competitive salary of $73,000 - $90,000 per year, depending on experience.

As a Home Health Care Professional with CarePartners, you will have the opportunity to develop meaningful therapeutic relationships with patients while preserving patient dignity and privacy. You will work within an extremely supportive and collaborative environment focused exclusively on quality patient care, where you can exercise your full scope of practice, pursue specialization, and champion innovative health care programs.

Responsibilities:

  • Developing and maintaining effective care plans for community health services
  • Working with patients to provide exceptional nursing services, both in-person and virtually
  • Providing services one-on-one with patients in a home setting
  • Communicating and liaising with patients, family members, and others within the interdisciplinary care team
  • Performing other clinical duties as required

Requirements:

  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • A strong desire and commitment to making a difference in the lives of our patients
  • Exceptional interpersonal skills
  • A valid driver's license and vehicle
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record, including 2-step TB test
  • Vulnerable sector check and a clear background check
