About the Role

We are seeking a skilled Nursing Professional to join our team at CarePartners. As a member of our community care team, you will have the opportunity to provide exceptional nursing services to patients in their homes.

Key Responsibilities:

  • Develop and maintain effective care plans with respect to community health services, independently or in cooperation with Ontario Health @Home (OH@Home).
  • Provide nursing services to patients in a home setting, either in-person or virtually.
  • Communicate and liaise with patients, family members, and other members of the interdisciplinary care team.
What You Bring

To be successful in this role, you will need to possess:

  • A Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma.
  • Current College of Nurses of Ontario (CNO) registration in good standing.
  • Exceptional interpersonal skills and a strong desire to make a difference in the lives of our patients.
  • A valid driver's license and vehicle, as regular travel is required.
  • Excellent English verbal and written communication skills, including CPR certification.
