We are seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team at CarePartners in the McDougall, McKellar, and Carling area of Ontario. As a community health nurse with us, you will have the opportunity to provide exceptional nursing services to patients in their homes, schools, clinics, workplaces, and retirement homes.
Overview

Salary
The estimated annual salary for this role is between $85,000 to $110,000, depending on experience, education, and qualifications. This competitive wage grid recognizes the value of your nursing skillset and expertise.
Job Description
As a community health nurse with CarePartners, you will be responsible for:
- Developing and maintaining effective care plans for patients receiving community health services;
- Providing exceptional nursing services to patients in their homes, schools, clinics, workplaces, and retirement homes;
- Communicating and collaborating with patients, family members, and interdisciplinary care teams to ensure seamless care delivery;
- Performing other clinical duties as required.
Requirements
To be successful in this role, you will need:
- A current College of Nurses of Ontario (CNO) registration in good standing;
- A strong desire and commitment to making a difference in the lives of our patients;
- Exceptional interpersonal skills and communication abilities;
- A valid driver's license and vehicle, as regular travel is required;
- CPR Certification and a current immunization record, including a 2-step TB test.
