Job Summary

We are seeking a skilled Mental Health Care Coordinator to join our team at Fraser Health Authority. As a key member of our interprofessional care team, you will play a vital role in providing person-centered and recovery-oriented care to patients with psychiatric and concurrent disorders.

Key Responsibilities
  • Screens psychiatric referrals from various sources, gathers background information, and determines service priorities.
  • Works closely with Emergency Department staff to identify potential psychiatric referrals and performs psychosocial assessments.
  • Develops care plans with final dispositions authorized by the Emergency Room Physician or consulting psychiatrist.
  • Provides general and crisis counseling to patients and their families, including short-term crisis follow-up according to approved treatment plans.
Requirements
  • Graduation from an approved school of Nursing or Psychiatric Nursing.
  • Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • One year of recent related experience in acute or community mental health and substance use settings.
