Job SummaryWe are seeking a skilled Social Worker to join our Mental Health and Substance Use team in Surrey, B.C. The successful candidate will be responsible for providing comprehensive therapeutic clinical counselling services to clients and their families, as well as coordinating services and managing cases.
Key Responsibilities:- Client Care: Provides therapeutic clinical counselling services to clients and their families, including crisis counselling and brief therapeutic interventions.
- Case Management: Coordinates services and manages cases, working collaboratively with interdisciplinary teams to ensure effective care planning.
- Assessment and Intervention: Conducts comprehensive social work and psychosocial assessments of clients and families, identifying high-risk factors and developing strategies for recovery.
- Educational and Support Services: Educates clients, families, and healthcare professionals on mental health issues, recovery, and community resources.
- Record Keeping: Maintains accurate and up-to-date client records, documenting assessments, charting client information, and preparing progress notes and reports.
- Community Collaboration: Collaborates with mental health and related community service agencies, family physicians, and other stakeholders to ensure seamless care coordination.
- Leadership and Development: Participates in program evaluation, identifies opportunities for change, and develops solutions to improve efficiency and effectiveness in service delivery.
- Education: Master's Degree in Social Work (MSW) from an accredited school of Social Work.
- Experience: Two years' recent, related experience in a mental health treatment setting or an equivalent combination of education, training, and experience.
- Licensure: Current full registration with the British Columbia College of Social Workers.
- Driving: Valid Class 5 BC driver's license with access to personal vehicle for business purposes.
- Communication: Excellent verbal and written communication skills, with ability to educate and provide consultation to clients, families, and healthcare professionals.
- Leadership: Ability to provide leadership to the interdisciplinary team in care planning and coordinate services effectively.
- Problem-Solving: Demonstrated ability to organize and prioritize workload in a complex environment, identifying solutions to improve efficiency and effectiveness in service delivery.
- Collaboration: Ability to work effectively with others, including working effectively in a multicultural environment, independently, and as part of an integrated interdisciplinary team.
