About the Role

As a Clinical Pharmacist at Fraser Health Authority, you will have the opportunity to work in a dynamic and fast-paced environment where you can make a real difference in the lives of our patients.

Job Summary

We are seeking an experienced Clinical Pharmacist to join our team. The successful candidate will be responsible for providing clinical pharmacy services, including individualized patient medication therapy, patient medication counseling, and consultation with physicians and other healthcare providers.

Responsibilities
  • Develop and implement therapeutic plans for patients based on their medical history, current health status, and treatment goals
  • Provide patient education and counseling on medication use, side effects, and potential interactions
  • Collaborate with healthcare teams to develop and implement drug therapy guidelines for individual disease conditions
  • Participate in quality improvement initiatives and contribute to the development of new policies and procedures
  • Maintain accurate and detailed records of patient care activities, including medication orders, dispensing records, and patient communication
Requirements
  • Current registration as a Pharmacist with the College of Pharmacists of British Columbia
  • Completion of an accredited Hospital Pharmacy Residency Program or equivalent combination of education and experience
  • Strong knowledge of evidence-based clinical pharmacy practice, medication use evaluation, and research processes and methodology
  • Excellent communication and interpersonal skills, with the ability to work effectively with healthcare teams and patients
  • Ability to prioritize tasks, manage multiple responsibilities, and maintain a high level of productivity in a fast-paced environment
