Job Title: Manager, Home and Community Care

Job Summary:

The Government of Nunavut is seeking a highly skilled and experienced Manager, Home and Community Care to join our team. As a key member of our healthcare team, you will be responsible for providing program expertise and coordinating support to planning, program development, service delivery, and evaluation for home and community care programs in the region.

Key Responsibilities:

  • Coordinate expertise across the department, in partner departments, and agencies to ensure efficient and effective program delivery.
  • Emphasize community development and deepening of community capacity to improve healthcare outcomes.
  • Ensure the efficient and effective coordination and support of Home and Community Care programs in compliance with territorial standards of care.
  • Deliver quality case management, nursing, and support services to clients in their home setting and other community settings.

Requirements:

  • Undergraduate degree in nursing or diploma in nursing with five years' experience in community nursing, including three years of supervisory experience.
  • Eligible to register with RNANTNU.
  • Basic BCLS certification with annual re-certification.
  • Wound care, diabetes care, palliative care, specialized foot care, peripheral intravenous (PIV), and central intravenous (IV) access certificates are considered an asset.

Recruitment Bonuses:

  • $5,000 on start date.
  • $5,000 at 18 months of service.
  • $10,000 at 30 months of service.

Retention Bonus:

  • $4,500 per year.
