- Develop and implement individualized Resident Care Plans in collaboration with the interdisciplinary team.
- Conduct timely assessments and observations of residents to ensure their physical, emotional, and social needs are met.
- Maintain a resident-centered environment that promotes dignity, respect, and autonomy.
- Supervise and mentor Health Care Aides/Personal Support Workers to ensure high-quality care is provided.
- Administer medications safely and effectively, following established protocols.
- Maintain accurate and comprehensive clinical records, including progress notes and care plans.
- Communicate effectively with residents, families, and colleagues to ensure seamless care coordination.
- Participate in education and training activities to enhance knowledge and skills in resident care.
- Contribute to a culture of safety and quality improvement by identifying and reporting concerns or incidents.
- Registered Practical Nurse certification from the College of Nurses of Ontario.
- CPR certification.
- Strong communication skills in English.
- Fully vaccinated against COVID-19.
- Ability to prioritize tasks and problem-solve in a fast-paced environment.
- Experience in Long Term Care is an asset.
