Job Summary

We are seeking a skilled Licensed Practical Nurse to join our multidisciplinary team at Horizon Health Network. As a key member of our team, you will provide holistic care to individuals and groups of patients, working to the full extent of the LPN scope of practice.

Key Responsibilities
  • Patient Care
    • Embody a culture of client-centered collaborative care
    • Develop and continuously evaluate individual nursing care plans
    • Engage in therapeutic relationships and support patients/families in recovery planning
    • Act as a patient advocate, promoting open communication and professionalism
    • Maintain knowledge of treatments, equipment, and supplies pertinent to the service
    • Collaborate with other team members and act as a role model
    • Complete assigned duties to ensure quality patient care provision
    • Provide educational sessions to patients and groups
    • Model a calm, respectful, and supportive environment
    • Supervise and facilitate client activities
  • Educational/Human Resources
    • Maintain personal certification skills required
    • Model professionalism and maintain standards of practice and professional boundaries
    • Assist with and support the orientation of new staff
    • Assist with educating patients, families, and staff
    • Support the preceptorship program
    • Promote and demonstrate an interest in program activities
    • Report unsafe practices or professional misconduct
  • Financial
    • Maintain awareness of fiscal responsibility and promote cost-effectiveness
    • Act as a change agent to enhance quality of care, work environment, and sustainability
    • Collaborate with the Manager regarding optimization measures
    Requirements
    • Minimum of 1 year of recent and direct experience in Addiction & Mental Health
    • Actively registered with the LPN Association of New Brunswick (ANBLPN)
    • Current BLS, Non-Violent Crisis Intervention, ASIST, and Motivational Interviewing certifications
    • High level of interpersonal skills and ability to work well within a team and independently
    • Demonstrated awareness of assessment tools and treatment modalities
    • Demonstrated knowledge of and adherence to relevant legislation and processes
    • Previous exemplary work and attendance record
    • Possess a valid driver's license
    • Able to fulfill the duties of the position
