Job Summary

The University Health Network is seeking a highly skilled and compassionate Registered Nurse to join our Diabetes/Endocrine Program as a Patient Educator. As a member of our multi-disciplinary team, you will play a vital role in providing patient education and support to individuals with diabetes, empowering them to manage their condition effectively.

Key Responsibilities

  • Provide patient education on diabetes management, including medication self-administration, insulin pump use, and glucose monitoring.
  • Adjust insulin and oral hypoglycemic agents using a Medical Directive, applying knowledge of nursing science and critical decision-making.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and support for patients with diabetes.
  • Participate in continuing education activities to stay up-to-date with the latest research and best practices in diabetes care.

Requirements

  • Current Registration with the College of Nurses of Ontario.
  • Basic Cardiac Life Support Certification.
  • Excellent interpersonal, communication, and decision-making skills.
  • Excellent organizational and time management skills.
  • Experience working in a fast-paced ambulatory setting with patients with complex medical needs.
  • Health assessment skills.
  • Experience with patients with diabetes, lipid disorders, thyroid disease, osteoporosis, and disorders of other endocrine glands.
