Mental Health Social Worker Opportunity

Mackenzie Health is seeking a highly skilled and compassionate Mental Health Social Worker to join our interdisciplinary team. As a key member of our team, you will provide psychosocial assessments, case management, advocacy, patient education, and community linkages to individuals with serious mental illness.

Key Responsibilities:
  • Provide comprehensive psychosocial assessments and develop individualized treatment plans
  • Collaborate with interdisciplinary teams to ensure comprehensive discharge planning
  • Provide education and support to patients and their families
  • Link with community resources to ensure seamless transitions
  • Contribute to program development and participate in research activities
Requirements:
  • Master's degree in social work from an accredited university
  • Member of the Ontario College of Social Workers and Social Service Workers
  • Minimum of 3 years of mental health experience
  • Experience working in psychiatric/mental health and with diverse ethno-cultural backgrounds
Preferred Qualifications:
  • Successful completion of CPI or other form of Management of Aggressive Behaviour training
  • Interest and demonstrated abilities in program planning & development and research
