Job Title: Social Worker, MSW

Job Summary:

Mackenzie Health is seeking a highly skilled and experienced Social Worker, MSW to join our Inpatient Mental Health team. As a key member of our interdisciplinary team, you will provide comprehensive psychosocial assessments, case management, and advocacy services to individuals with serious mental illness.

Key Responsibilities:

  • Conduct thorough psychosocial assessments to identify client needs and develop individualized care plans.
  • Provide case management services, including linking clients with community resources and coordinating discharge planning.
  • Advocate for clients' rights and interests, ensuring their needs are met and their voices are heard.
  • Collaborate with the interdisciplinary team to provide comprehensive care and support to clients.
  • Develop and implement social work best practices within the team environment.

Requirements:

  • Master's degree in social work from an accredited university.
  • Member of the Ontario College of Social Workers and Social Service Workers.
  • Minimum of 3 years of mental health experience.
  • Experience working in psychiatric/mental health and with individuals from diverse ethno-cultural backgrounds.

Preferred Qualifications:

  • Successful completion of CPI or other form of Management of Aggressive Behaviour training.
  • Interest and demonstrated abilities in program planning & development and research.
