Job Title: Social Worker, MSW

Job Summary:

Mackenzie Health is seeking a highly skilled and experienced Social Worker, MSW to join our Inpatient Mental Health Program. As a key member of our interdisciplinary team, you will provide comprehensive psychosocial assessments, case management, and advocacy services to individuals with serious mental illness.

Key Responsibilities:

  • Conduct thorough psychosocial assessments to identify client needs and develop individualized care plans.
  • Provide case management and advocacy services to ensure seamless transitions between hospital and community-based care.
  • Collaborate with interprofessional teams to develop and implement care plans that promote patient-centered care.
  • Develop and maintain relationships with community resources and service providers to ensure access to necessary support services.
  • Participate in program development and research activities to enhance the quality of care and services provided.

Requirements:

  • Master's degree in Social Work from an accredited university.
  • Member of the Ontario College of Social Workers and Social Service Workers.
  • Minimum of 3 years of mental health experience, preferably in an inpatient setting.
  • Experience working with individuals from diverse ethno-cultural backgrounds.

Preferred Qualifications:

  • Successful completion of CPI or other form of Management of Aggressive Behaviour training.
  • Interest and demonstrated abilities in program planning and development, research, and education.
