Job Title: Social Worker, MSW

Job Summary:

Mackenzie Health is seeking a highly skilled and experienced Social Worker, MSW to join our team. As a key member of our interdisciplinary team, you will provide assessment, case management, advocacy, patient education, and community linkages to individuals with serious mental illness.

Key Responsibilities:

  • Provide psychosocial assessments and interventions in response to the unique and complex needs of each client.
  • Develop and implement comprehensive discharge plans in collaboration with the interdisciplinary team.
  • Link with other service providers in the hospital and community to ensure seamless transitions and support for clients.
  • Contribute to program development and participate in research activities to enhance the functioning of individuals with mental illness and their families.

Requirements:

  • Master's degree in social work from an accredited university.
  • Member of the Ontario College of Social Workers and Social Service Workers.
  • Minimum of 3 years of mental health experience.
  • Experience working in psychiatric/mental health and with individuals from diverse ethno-cultural backgrounds.

Preferred Qualifications:

  • Successful completion of CPI or other form of Management of Aggressive Behaviour training.
  • Interest and demonstrated abilities in program planning & development and research.
