About the Role
The Licensed Practical Nurse (LPN) is a key member of our care team, responsible for coordinating delivery of care to residents in accordance with their individualized care plans. This role requires a strong commitment to resident-centered care and a collaborative approach to working with other healthcare professionals.
Key Responsibilities
Assess residents’ needs and implement and evaluate individualized resident-centred care plans.
Ensure residents’ rights are observed and adhere to a holistic philosophy of wellness.
Liaise with physicians, community agency representatives, families, visitors, volunteers, other disciplines and departments to ensure quality resident-centered care is delivered.
Assess and treat all wounds on the unit and act as a resource to care staff for strategies to prevent skin breakdown. Consult with RN or designate for changes in treatment strategies as needed.
Administer prescribed medications and treatments within the limitations of the BC College of Nurses and Midwives (BCCNM).
Monitor and report changes in physical and psychological conditions of residents. Document changes on the resident health record, notifies family and physician of changes in condition.
Active participant in Interdisciplinary Care Conferences.
Participate in care plan reviews to ensure current and individualized care plans.
Supports the administrative requirements at the community in accordance with established standards, including but not limited to; admissions, discharge and ensuring confidentiality is maintained at all times.
Actively participates in orientation of new staff, community programs, meetings, committees and educational in-services.
Provides coaching and direction to care staff as needed and uses in the moment education to improve resident care which includes completing all Point of Care (POC) documentation. Seeks assistance/support from RN or designate as needed.
Maintains an up-to-date knowledge of community policies and procedures, particularly those concerning emergency procedures including equipment management, guidance and assistance on safe work practices.
Performs other related duties as required.
Qualifications and Skills
Graduate of an approved program for Practical Nursing and holds a current license with the British Columbia College of Nurses and Midwives (BCCNM) as an LPN.
Valid Occupational First Aid Level 1 certification.
Successful annual Resident Assessment Instrument (RAI) competency testing.
Previous experience in a long term care facility an asset.
All successful applicants must pass the vulnerable sector Criminal Record Check applicable to Provincial guidelines (see BCCNM license).
Ability to read, write, and speak English proficiently and understand verbal and written instructions.
Computer skills including the ability to navigate basic computer programs such as Word, Excel, Outlook.
Demonstrates collaborative and team working skills.
Must be in good health, physically able to perform repetitive tasks, lift, squat, bend, stand and walk as required to perform the duties of the position.
