We are seeking a Clinical Pharmacist to join our team at Pillway BC, where you will play a vital role in providing patient coaching and management for medication checks, diabetes management, asthma, COPD, and other health programs.

Responsibilities
  • Practice within provincial guidelines and uphold the Code of Ethics and Standards of Practice.
  • Provide comprehensive counseling on all prescriptions and health topics to ensure positive patient outcomes.
  • Perform expanded scopes of practice, including minor ailments, medication reviews, and pharmaceutical opinions.
  • Conduct detailed medication checks and patient coaching sessions, particularly focusing on chronic disease management (diabetes, asthma, COPD).
  • Collaborate effectively with allied healthcare professionals to ensure holistic patient care.
  • Maintain accurate records in accordance with legislative requirements.
  • Actively engage in continuous professional development and training programs to stay current with industry trends and best practices.
  • Develop and implement individualized care plans based on patient needs and health goals.
  • Educate patients on lifestyle modifications, medication adherence, and self-management techniques.
  • Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.
Qualifications
  • A valid license and registration with the Ontario College of Pharmacists.
  • Excellent verbal and written communication skills.
  • Strong interpersonal skills with the ability to build rapport and trust with patients.
  • Proficiency in using healthcare technology for patient management.
  • Proven problem-solving skills with a patient-centric approach.
  • Ability to work collaboratively within a multidisciplinary healthcare team.
  • Experience in patient coaching and chronic disease management is an asset.
  • Strong organizational skills and attention to detail.
  • Ability to multitask and manage time effectively in a fast-paced environment.
  • Demonstrated commitment to patient safety and quality improvement initiatives.
  • Knowledge of current pharmacological and therapeutic principles.
  • Capability to provide compassionate and empathetic patient care.
  • Ability to educate and mentor pharmacy team members and patients.
