We are seeking a skilled and compassionate Dedicated Chronic Disease Nurse - Diabetes Management Specialist to join our team at Fraser Health Authority in Delta, B.C.

Salary and BenefitsThe estimated salary for this position is $41.42 - $59.52 per hour, offering a competitive compensation package that includes benefits and opportunities for career growth and advancement.
Key Responsibilities- Develops, implements, and evaluates individualized care plans for clients with diabetes, working collaboratively with other healthcare professionals.
- Provides education and support to patients and their families on diabetes management, empowering them to take control of their health.
- Conducts outpatient assessments and shares information with other healthcare team members to ensure quality care.
- Participates in clinical case reviews, program development, and evaluation, contributing to the improvement of patient outcomes.
- Acts as a resource to other healthcare professionals, providing guidance and support on diabetes-related matters.
To be considered for this role, you must have:
- A degree in nursing from an approved school of nursing.
- At least two years' recent experience in a clinical setting, assessing and treating patients with diabetes.
- Certification as a Certified Diabetes Educator (CDE) or equivalent.
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
