Job Summary

We are seeking a highly skilled and dedicated Regional Indigenous Health Coordinator to join our team at the Winnipeg Regional Health Authority. The successful candidate will be responsible for coordinating discharge planning for Indigenous patients throughout the WRHA in a professional manner.

Key Responsibilities

  1. Support and participate in regional utilization initiatives.
  2. Collaborate with other health professionals to coordinate discharge planning for Indigenous patients.
  3. Demonstrate effective communication and teaching skills with Indigenous patients and hospital staff.
  4. Participate in professional development activities to enhance knowledge and skills.

Requirements

  • Minimum of 2 years recent experience working with Indigenous patients in an acute care setting.
  • Extensive experience working in and with the Indigenous community and familiarity with available resources.
  • Bachelor's degree in a health or social services discipline or equivalent combination of education and experience.
