Clinical Care Coordinator

About the RoleThis position is responsible for coordinating and providing quality care to patients in their own residence or alternate care settings. You will work closely with clients, their families, and other healthcare professionals to ensure that patients receive safe and effective care.
Key Responsibilities- Care Coordination: Work with clients, families, and healthcare teams to develop and implement individualized care plans.
- Nursing Services: Provide a wide range of nursing services, including assessments, interventions, and education to patients and their families.
- Communication: Communicate effectively with clients, families, and healthcare teams to ensure seamless care coordination.
- Leadership: Support an integrated and holistic approach to patient care, health promotion, and maintenance.
To be successful in this role, you will need:
- A minimum of 5 years' experience in acute care or senior's health.
- Completion of an accredited nursing education program.
- Active registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
- Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.
- Free online courses may be required for those who have not practiced as a Registered Nurse within the last five years.
