About the Opportunity
The Case Manager for the Heart to Home Program is responsible for leading, developing, and supervising all care planning activities for children and youth in The Link Healing Home. This role ensures care providers are supported in delivering quality care and contributes to the continuous development of the program.
Key Responsibilities:
- Develop and implement holistic care plans that meet the unique needs of each child and youth.
- Collaborate with internal and external stakeholders to ensure seamless transitions and placements.
- Provide guidance and support to care providers to ensure quality care is delivered.
- Contribute to the development and implementation of program policies and procedures.
- Participate in clinical consultation and team meetings to ensure best practices are shared and implemented.
Requirements:
- Post-secondary degree in a human service field, with a focus on child welfare or a related field.
- 3 years of experience in child welfare, foster care, or residential care.
- Strong analytical and critical thinking skills, with the ability to engage and plan with youth.
- Valid 5F driver's license, with a current driver's abstract and proof of vehicle insurance.
- Strong written and oral communication skills, with the ability to work effectively in a team environment.
