Case Manager

The Link is seeking a highly skilled Case Manager to join their team in Thompson, MB. This is a full-time position that offers a competitive salary and a dynamic work environment.
Key Responsibilities:
- Collaborate with youth, families, and care providers to develop and implement holistic care plans.
- Ensure the smooth transition of youth from one care setting to another.
- Conduct regular home visits and meetings with foster parents to assess the quality of care.
- Develop and maintain relationships with internal and external stakeholders to ensure the best possible outcomes for the youth.
- Participate in clinical consultations and contribute to the development of the agency's therapeutic model of care.
Qualifications:
- Bachelor of Social Work degree or equivalent experience.
- Minimum 3 years of experience in child welfare, foster care, or residential care.
- Strong analytical and critical thinking skills, with the ability to engage and plan with youth.
- Valid 5F driver's license and satisfactory driver's abstract.
- Strong written and oral communication skills.
- Microsoft Office skills at an intermediate level.
- Satisfactory criminal record check and child abuse registry check.
