Job SummaryWe are seeking a skilled Registered Nurse to join our team as a Healthcare Professional - Community Care Specialist. In this role, you will provide high-quality care to clients in the downtown Toronto area, working collaboratively with a supportive team.
Key Responsibilities:- Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and client needs.
- Support clients through care transitions by collaborating and connecting them with community support services.
We offer a competitive salary of $120,000 per year, paid per client visit. This model relies on your professional judgment for client care, ensuring you have the autonomy to make decisions that benefit our clients.

You will be required to work days, evenings, nights, and every other weekend. A 24-month commitment to a full-time work schedule is expected, along with a commitment to not simultaneously receive funds from the Ministry of Health for the Tuition Support Program for Nurses.
