As an Occupational Therapist at VHA, you will play a vital role in assessing and providing consultation, recommendations, and interventions to adults and geriatric groups of clients in their homes using evidence-based best practices. You will work collaboratively with the client care team to ensure coordinated delivery of care, problem-solving, and promoting independence.
**Key Responsibilities:**
- Assessing and implementing occupational therapy interventions in accordance with professional practice guidelines and standards established by VHA and relevant regulatory colleges.
- Implementing client interventions based on assessment information, client input, and integrated care plans.
- Ensuring services provided are best practice, evidence-based, and evaluated through routine use of valid and reliable outcome measures.
- Maintaining open communication with all members of the client's team regarding plans and schedules for interventions.
- Documenting all assessment findings, treatment plans, progress, and discharge summaries of interventions as required.
**Required Qualifications:**
- Degree in Occupational Therapy from a recognized school of Occupational Therapy.
- Member in good standing with the College of Occupational Therapists of Ontario.
- Valid Driver's License with access to a vehicle (asset).
- Compassionate and empathetic individual with excellent interpersonal skills.
- Ability to work independently and manage caseload effectively.
- Skilled in electronic documentation and able to provide virtual care.
- Registered ADP authorizer (asset).
