Key Responsibilities:- Provide direct clinical care to patients, including assessment, diagnosis, and treatment.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with healthcare professionals to ensure seamless care coordination and communication.
- Conduct health education and promotion activities to empower patients and families to take an active role in their health.
- Participate in quality improvement initiatives to enhance patient outcomes and experiences.
- Current registration with the College of Nurses of Ontario.
- Bachelor of Science in Nursing (BScN) degree.
- Nurse Practitioner Certification.
- Minimum three (3) years' experience in advanced practice nursing in primary care.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
