Nurse Practitioner

Orillia Soldiers' Memorial Hospital (OSMH) is seeking a highly skilled and experienced Nurse Practitioner to join our Primary Care team. As a Nurse Practitioner - Primary Care Specialist, you will play a vital role in providing high-quality, patient-centered care to our community.
Key Responsibilities:- Provide comprehensive primary care services to patients, including assessment, diagnosis, treatment, and management of acute and chronic conditions.
- Work collaboratively with the healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
- Conduct home visits to assess and manage patients' health needs in their own environment.
- Consult with specialists and other healthcare professionals as needed to ensure comprehensive care.
- Participate in quality improvement initiatives to enhance patient outcomes and experiences.
- Current registration with the College of Nurses of Ontario.
- Bachelor of Science in Nursing (BScN) degree.
- Nurse Practitioner Certification.
- Master's degree in Nursing or Nurse Practitioner Stream (preferred).
- Primary Care Nurse Practitioner Certification (preferred).
- Minimum three years' experience in advanced practice nursing in primary care.
- Minimum three years' relevant clinical specialty-related experience as a Registered Nurse.
