Key Responsibilities:

  • Provide direct clinical care, education, research, consultation, and treatment to support patient care needs.
  • Establish relationships with physicians, nurses, discharge planners, and other OSMH staff to enhance integrated care.
  • Work actively with the COHT Care Clinic team to prioritize patient care needs.
  • Visit patients in their homes for assessment, treatment, and advanced clinical management as needed.
  • Be the Most Responsible Practitioner (MRP) for patients whose care needs can be met within the NP scope of practice.
  • Consult as needed with Hospitalists, Specialists, Community Primary Care Practitioners, and Allied Health team members.
  • Assess patients/families through multiple strategies, including comprehensive admission history and interview, advanced physical examination, interval assessments, and the interpretation of diagnostic testing.
  • Utilize diagnostic reasoning and decision-making process to formulate a list of hypotheses, determine the need for, order, and interpret diagnostic testing, and formulate a medical diagnosis within the specialty scope of practice.
  • Communicate significant clinical findings to consulting physicians and facilitate ongoing communication of patient progress and plan of care to the healthcare team.
  • Initiate/orders, tailor, and evaluate/discontinue selected medical and nursing therapy within the specialty scope of practice.
  • Perform specialty-specific diagnostic and therapeutic interventions in accordance with scope of practice, approved medical directives for controlled acts, and established practice guidelines.
  • Facilitate patient care by consulting with the patient/family and inter-professional patient care team to formulate, initiate, coordinate, and evaluate a comprehensive and collaborative patient-centered plan of care.
  • Promote continuity of care within the Couchiching Ontario Health Team by initiating and coordinating patient discharge plans, warm transfers, and integrated care methodology.
  • Write/dictate consultations, progress notes, and discharge summary reports.
  • Evaluate patient care outcomes and nursing practice issues to ensure the highest possible quality of care.
  • Supervise/precept nursing students (graduate and undergraduate) and other interdisciplinary students as required in their clinical experience and evaluate their performance.
  • Provide clinical nursing leadership to promote professional development to assist nursing staff in the application of knowledge/theory to practice through role-modeling and coaching, and by participating in formal staff education programs in collaboration with the program leadership team.
  • Provide leadership in the critical analysis and review of literature and the application of research findings to practice through the development and implementation of clinical practice guidelines and standards of care.
  • Identify and implement strategies to translate and interpret research findings and theoretical frameworks into practice.
  • Facilitate the utilization of research by nursing and inter-professional staff and contributing to knowledge base through presentation or publication.
  • Support program planning, implementation, and evaluation as well as project management.
  • Safety and risk management.
  • Participating in a supportive and healthy working environment.
  • Perform other duties as may be assigned by program leadership.

Qualifications:

  • Current registration with the College of Nurses of Ontario is required.
  • BScN required.
  • Nurse Practitioner Certification required.
  • Masters degree in Nursing/Nurse Practitioner Stream is preferred.
  • Primary Care Nurse Practitioner Certification preferred.
  • BCLS is required.
  • Minimum three (3) years' experience in advanced practice nursing in primary care required.
  • Minimum three (3) years relevant clinical specialty-related experience as a Registered Nurse required, experience in Primary Care preferred.
  • Recent experience in a leadership role within an inter-professional team preferred.
  • Relevant knowledge of integrated care environment preferred.

  • Demonstrated commitment to the concepts of Continuous Quality Improvement and Customer Service.
