VHA Home HealthCare is seeking a skilled Registered Practical Nurse (RPN) to join our team as a MGH2Home Program Coordinator. As a key member of our team, you will play a vital role in providing high-quality, client-centered care to individuals and families in their homes, schools, and community settings.
Key Responsibilities- Assess, monitor, and make care planning/coordinating decisions to ensure the best possible outcomes for clients.
- Evaluate care plan interventions through ongoing reassessment and evaluation results.
- Ensure the quality of care by identifying and acting on factors that affect practice autonomy and delivery of quality care.
- Demonstrate professionalism, leadership, judgment, and accountability in interactions with clients and families.
- Support client safety and care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
- Inform nursing supervisors of client activities and relevant concerns or changes in the client or the client care plan.
- Registered and in good standing with the College of Nurses of Ontario (CNO).
- Membership of your Professional Association preferred (WeRPN).
- RPN diploma/degree from a recognized university or college.
- Valid First Aid or CPR Certification.
- Acute Care and Medical-Surgical nursing experience is an asset.
- Competency in IV Infusion is preferred.
- Knowledge of community nursing is an asset.
- Demonstrate effective communication (written and verbal) and documentation skills.
- Demonstrate reliable attendance, punctuality, and availability for the role.
- Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.
