About the Role:
  • Provide primary care services to clients with enhanced care needs who are involved in the criminal justice system.
  • Work collaboratively with the FACT team to develop and implement individualized care plans.
  • Contribute to the development and implementation of clinical policies and procedures.
  • Participate in quality improvement initiatives and contribute to the growth and development of the FACT team.

Requirements:
