Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our Transitional Care team at Homewood Health Centre in Guelph, Ontario. As a key member of our interdisciplinary team, you will provide high-quality patient care, collaborate with colleagues, and contribute to the development of innovative mental health programs.

About the Role

In this dynamic role, you will be responsible for assessing, planning, developing, and coordinating care for individuals in the North Wellington area. You will work closely with patients, families, and healthcare professionals to deliver seamless services and support.

Key Responsibilities
  • Assess patients' physical and emotional needs, develop personalized care plans, and implement evidence-based treatment strategies.
  • Collaborate with multidisciplinary teams to ensure comprehensive care and effective communication.
  • Provide education and support to patients and their families on mental health issues, wellness strategies, and disease management.
  • Participate in quality improvement initiatives, program development, and research activities to enhance patient outcomes and advance practice excellence.
Requirements

To be successful in this role, you will possess:

  • A current registration with the College of Nurses of Ontario (CNO) as a Registered Nurse (RN).
  • A minimum of 2 years of recent experience in emergency mental health/acute assessment, preferably with a Mental Health Nursing certificate.
  • Strong assessment skills, critical thinking, and decision-making abilities.
  • Excellent communication, problem-solving, conflict resolution, and organizational skills.
  • Proven ability to work independently and collaboratively as part of an interdisciplinary team.
  • CPR/BLS certification and up-to-date knowledge of Mental Health Act and community resources.
