Job Description

We're seeking an experienced Advanced Mental Health Nurse Practitioner to join our team at Homewood Health Centre. In this role, you'll be working with a collaborative, interdisciplinary team to deliver advanced practice nursing to patients and families with complex situations.

You will:

  • Utilize your comprehensive theoretical knowledge base and advanced clinical competence to provide holistic care to patients, including health assessments, diagnosis, and development of personalized care plans.
  • Work collaboratively with consulting physicians to facilitate early assessment and intervention for patients requiring consults.
  • Support the management of patients through comprehensive nursing/medical history, physical and psychosocial assessment of patients' status.
  • Use established care paths and order sets to guide care practices for patients from admission through to discharge.
  • Develop and maintain individualized treatment and care plans for complex patients in collaboration with physicians and psychiatrists.
  • Assist in coordinating care with patients, families, and communities.
Required Skills and Qualifications

To be successful in this role, you'll need:

  • A minimum of 3 years of experience as a NP, preferably in mental health and addiction expertise.
  • Current registration in the Extended Class (Nurse Practitioner) with the College of Nurses of Ontario.
  • Active membership in related professional associations, such as NPAO and RNAO.
  • Mental Health Nursing certificate preferred.
  • Masters of Nursing required.
  • Innovative and creative approach to problem-solving.
  • Excellent assessment and clinical skills.
  • Ability to prioritize and organize multiple demands.
  • Autonomy in decision-making.
  • Working knowledge of the Mental Health Act and community mental health resources.
Salary Information

The estimated salary range for this position is $80,000 - $110,000 per annum, depending on qualifications and experience.
