Nurse Liaison

The Transition Liaison Registered Nurse plays a vital role in enhancing the quality of life for patients and their families by providing comprehensive nursing care and facilitating communication linkages between healthcare services, community partners, and patients.
Key Responsibilities:
• Provides nursing care to patients, working independently and as part of an interdisciplinary team to identify and address patient needs and eligibility for services.
• Collaborates with healthcare providers to develop and implement patient care plans, ensuring seamless transitions between healthcare services and facilities.
• Demonstrates continuous improvement and evidence-based practice in nursing care, maintaining and advancing clinical competence to deliver high-quality patient outcomes.
• Plans, implements, and evaluates patient access and care transition activities, ensuring effective communication and coordination between healthcare services and facilities.
• Provides comprehensive assessments of patients, identifying variances from care/discharge plans and service needs, and resolving patient, caregiver, and system issues.
This is a casual opportunity with Interior Health, offering flexibility and the potential for regular employment. As a casual employee, you will be eligible to apply for internal postings, including permanent and temporary positions.
