About the Role:

Key Responsibilities:

  • As a Licensed Practical Nurse (LPN) - Transition Liaison, you will practice in alignment with the organization's vision and values, adhering to the professional standards and code of ethics established by the British Columbia College of Nurses and Midwives (BCCNM) while focusing on patient-centered care.
  • You will play a key role in planning, implementing, and coordinating patient access and care transitions across different healthcare services within IH facilities, utilizing your expertise in complex care planning and addressing the health service needs of assigned patients.
  • As a resource to patients, families, and healthcare professionals, you will assess needs and determine eligibility for services and resources, providing exceptional support and guidance.
  • Operating within the full scope of practice outlined by BCCNM, you will uphold competency guidelines and follow organizational protocols and standards, ensuring the highest level of patient care.

Flexibility and Opportunity: This position offers flexibility as a casual opportunity, allowing for variable hours and short-notice assignments. As a casual employee, you can apply for internal postings, including permanent and temporary roles, and may have the opportunity to progress to regular positions.

Work Schedule: Shifts for this position are scheduled during the day, rotating between 08:00 to 16:00, and 8:00 to 16:33 hours.
